Need Help with ESIC Form 1: Can I Submit a Printout for New Employee Declarations?

I need a Declaration form, i.e., Form 1 for ESIC in the 17-digit format. Also, I would like to know if ESIC accepts the printout/xerox of the same at the time of submitting the declaration of a new employee.

They accept a xerox copy of the declaration form. You can obtain the declaration form from the ESIC website.

No, they are not accepting the Xerox copy because they returned my forms that I had submitted in Xerox.

Secondly, I didn't find the 17-digit format on the ESIC website.

There are two ways for submitting ESIC form of a new employee:

1) Along with the declaration form, fill out the other form which is required for taking a photograph and submit it to the local ESIC office.

2) Fill the declaration form in duplicate or take a copy of the filled declaration form and submit the same to the local ESIC office.

Both ways are acceptable.

Please find attached the ESIC new form. They will only accept the declaration form as a xerox copy if it is presented on legal paper; it will not be accepted on A4 paper. Kindly ensure it is on legal paper size before submission.
