Please find PT Form I & II registration for your information. Kindly fill in the necessary details and submit the same along with the following documents:

1) MOA & AOA.
2) Company PAN Card.
3) Proof of your HYD branch address (preferably BSNL Landline).

Also, please verify with your HYD staff the location of your PT branch.
