Need Help Creating a Manager Appraisal Questionnaire: How to Measure Performance in IT and Creative Roles?

Hi Guys,

I need help in setting a questionnaire for the appraisal of Managers. A format form with the likely to be asked questions for Managers and how the ratings should be calculated. Also, I work for an IT Value-Added Services Firm, and we have Business Development guys and creative guys, so how can their performance be measured or KRA set for them, please explain. The sample form for Manager appraisal would be of great help.

Hi,

My boss has also told me to create a questionnaire-type appraisal form for the managers, which I have prepared. However, he is still not satisfied with it and wants me to add more questions to effectively evaluate them. I would appreciate it if someone could provide suggestions for additional questions.

Some of the questions I have included in the current format are:

- Define your KRAs.
- How was your past year for you and why?
- What was your most important achievement?
- Mention the most difficult element of your job.
- What elements of your job interest you the most?
- How do you handle criticism? Please explain with an example.
- How many leaves have you taken in the past 6 months?
- Do you consider suggestions from your subordinates? Provide an example of a recent suggestion you have implemented.
- What was your most successful presentation in the past 6 months and why?
- How do you coach and develop your subordinates? Explain with an example.
- Describe your recent team-building exercise.
- Outline your recent strategies and directions taken in favor of the company or employees.
- What are your goals for the next year?
- What support do you expect from your boss to enhance your performance in your current position?
- In which areas do you feel you need training and why?

I hope these revisions help in making the appraisal form more comprehensive and satisfactory. Thank you for your assistance in this matter.

Please do not request finished projects or questionnaires on this forum without giving enough information. What's the project for? Give more information about yourself and the course, if any. Have you done any literature survey and formed your aim and objectives for the research? You also need a conceptual framework for that. Do you have any hypotheses to test? If so, could you please state them so that you can get a focused answer. If you have not done any of the above essential preparation for conducting a project, I would advise you to do so and then pose a precise question.
