How Do I Implement a 180-Degree Appraisal as the Sole HR in My Company?

I am Chandana, working as an HR Executive in a software company. I am the only HR in my company who is taking care of all things. As a recruiter in the past, I have experience in conducting performance appraisals. But I can manage it if you guide me. We have decided to implement 180-degree appraisal as this is the first time. So, please guide me on how to proceed and what documents I require.
