ESI:
Form 1 - Declaration
Form 5 - ESI returns
Form 6 - Consolidated returns register

EPF:
Form 2 - Declaration
Form 5 - Addition
Form 10 - Deletion

Just a request, please use the EPF Form 2 Declaration (Revised form).

For the purpose of EPF forms - 2 (nomination), 5 (new joining), 10 (detention), 13 (transfer), combined form, 12A (monthly), and 6A, 3A for annually are submitted.
