Can anybody please assist me in knowing what are the information one needs to furnish in an EMP ID card.
Is mentioning designation important ?

hi pfa a sample, u also print the tearm & condition back side of it like, its a property of --- company & if someone find it drop in post ,u mention their address & cont no also.

2. Blood Group
3. Clear photo
4. Address to which card is to be returned if found
