Seeking Sample Employee Surveys for Honest Feedback on Management and Morale

I am the director of HR for a non-profit organization and would like to know if anyone has a sample of an employee survey. Employees would anonymously answer questions about how they feel about management, the company (morale, vision, etc). I have compiled a little bit from the 360 appraisal but am looking for something more.
