5. Canteen / Transport Facility
6. Any new policy implemented
prepare a questionnaire on the above heading or accoring to your company welfare messures, policy you can choose heading

Compensation and Benefit
Performance Management System
Culture and TEam Building
Leadership
Policies and Programmes
Change Management
Leaves & Attendance
And any other areas which u feel would affect the employee morale and motivation.
