Can someone provide the best practice KPI format or template?

One thing you need to keep in mind that KPIs depend on the following:
• Job description
• Business Requirement of the organization based on the annual Business plan which are cascaded to
• Division Objectives & KPI to
• Department objectives & KPI to
• Individual Objectives & KPI
Therefore you cant just give us the designation and create a KPI based on it there are a lot of factors to be considered. You also need to bare in mind that those KPI have to be measurable.
