Annual PF Return Submission

Dear Seniors,
Please give me the proper guidance for submission of annual PF returns what are the documents required for this returns and one more thing is if I didn't submit the employee details at the beginning time then what can I do in this situation,kindly give me the right suggestion .

Form 3A,6A ALONG WITH ORIGINAL PF CHAALAN OF CORRESPONDING YEAR,ONE COVERING LETTER,AND COPY OF 6A IN .DBF FORMET

Relaxation in payment of employers share of contribution of ESI (4.75% to 3 %) for new establishment which are covered for the first time w.e.f. 01.05.2016 from which month they can start claiming. Can it be claimed for 24 months from Jan 2017. Please clarify on the same.
