PF Withdraw Process - Pf Transfer

DAER ALL,
PREVIOUSLY I WARKED AS A AGENCY MANAGER OF BHARTI AXA LIC IN KOLKATA KANKURGACHI BRANCH.
My Emp Id i s : 18897
i leave the organization long , but company given a due notice of RS- 981/-.
so, how can i attained my PF.
