Need Help Creating an Effective Performance Appraisal Format for MBA and MCA Faculty Members

The management of my college has given me the responsibility to create a performance appraisal format for MBA and MCA faculty members. Please assist me with this task. Kindly suggest how to structure the points for the appraisal.

Dear Sonika,

What subject do you teach? Unless one knows all the duties the faculty have to perform, it is not easy to give you a format. For example, faculty can be assessed on 3 aspects: Teaching, Research, and service.

I teach HR and OB-related subjects to MBA students. Teaching and working in HRM area is my passion. Actually, a few points were in my mind on how to prepare a faculty appraisal form, but I still wanted to seek suggestions from seniors. I did search CiteHR, but couldn't find this specific form.
