Please tell me the process to apply for a duplicate PAN card. I have my PAN card number; what else do I need?

The procedure to apply for the duplicate PAN card is as follows:

You will have to submit a newly filled Form 49A and a photocopy of the old PAN card along with valid ID proof and address proof at any PAN centers.
