Questionairre To Evaluate Employee Performance

Hi,
I'm currently doing a research on 'The relationship between leadership style and employees' performance" on quantitative method. I'm going to measure the leadership style using the MLQ questionnaire.Therefore, now I require a questionnaire to measure employee performance.Would anyone be able to provide it?I need questionnaire used by past researchers.Thank you.
