Since your service is less than or up to 6 months, you are eligible to withdraw only your and your employer's contribution to the PF. First, check your UAN number as provided by your employer. If they have not provided it to you, you can check and get your UAN number from the link

To withdraw PF without the involvement of your employer, you have to fill out the UAN-based Form 19 and send it directly to your concerned PF office along with Form 15G, a Xerox copy of your PAN card, NEFT form, and a cancelled cheque in support of your KYC.
Please note that the process will be applicable to those who have activated their UAN and have their KYC details uploaded and approved by the employer in the PF web portal.
