Need Help Designing an HR Policy Survey Questionnaire – Can You Share a Sample?

I was trying to design a questionnaire to conduct an HR Policy Survey. Can someone please help me out with the design? It would be great if someone could share a sample questionnaire with me.

What is the objective of your survey? Are you incorporating a survey to help you understand the satisfaction levels among employees regarding your policies, or is it an industry-wide survey? Which industry vertical do you belong to? Based on this information, perhaps we may be able to shed more light.

Hi,

The purpose of the survey is to evaluate the existing HR policies of the company and try to understand the progressive policies adopted by other companies. I am conducting this survey for a manufacturing company. I am planning to conduct this survey across industries and it is not being used to gauge the satisfaction levels of employees within the company.
