Need Help with PF Withdrawal: How to Get an Affidavit for Unclaimed Account?

My account is in “UnClaimed” state because I didn't withdraw the PF funds within 3 years (I am not currently contributing to that account). Now when I tried to withdraw the funds, I was told to submit an affidavit because my account is in an UnClaimed state, can someone help me in getting the sample template of the affidavit?

You can ask for the format from your employer or the PF office where you are due to withdraw.

I believe you should re-check with the PF office. I think a normal letter from you indicating the reason for not withdrawing, along with a letter from the employer, will work. Please attach these letters along with your Settlement forms.
