Query About Form 11 Under Pf Act, 1952 - DOC Download

Dear all , i am working with one company. My query is what is the use of Form No. 11 under PF Act. 1952. My HR exceutive fill that form for most of the employees and i dont know about that form. Please send the form details.

The form 11 is declaration by the member for his membership of PF.
If you view the form 11 you will understand the necessity of this form.
Member declare if he has withdrawn his earlier PF, His Previous PF no EPF and EPS. or He was not member of EPF.
If he has this 1st employment, and PF wages above Rs 15 K, He will not be member of EPS scheme.
Hence the declaration is important

In Form No. 11 under PF Act. 1952 mention Scheme Certificate. What is the purpose we are using Scheme Certificate. And also attach the Specimen of Scheme Certificate.

Scheme certificate:
If member is not working any establishment where PF is applicable may take Scheme Certificate for EPS. In the Scheme Certificate your Service details before Nov 1995 and after Nov 1995 are mentioned.
As well as other family details and last 12 months average salary and salary on Nov 1995 is mentioned.
With the certificate you can claim Pension on attending age 50/58, subject to fulfillment of other conditions, or can withdraw the EPS amount.

I am working in a construction company as daily labor and my basic wages is 11540 and they deduct 12% pf in basic wages .Now they asking me to fill the form 11 ,what is usage of form 11.After giving that from what happen,can I claim my PF amount.Kindly give advise to me can I fill or Not.

Dear Mr. Dhinesh,
The form 11 is declaration given by the member while joining the establishment.
In the declaration the details of your previous establishment are given along with code no and PF no etc.
If company wants to update their record, which they have not done earlier, fill the form 11 and submit to Company.
