New to Induction Training Research: How Do I Create an Effective Evaluation Questionnaire?

Hi,

I am a very new member. I'm doing a project about induction training. For that, I want to prepare a questionnaire to critically evaluate the induction training conducted in a large organization. Can anybody please help me to do it?
