Dear Namtha,

You have to collect Form-A (See Rule 12(4)) of the Tamil Nadu Labour Welfare Fund Rules 1973. We are getting the Form-A from The Secretary, Tamil Nadu Labour Welfare Board, P.B. No. 718, DMS Compound, Teynampet, Chennai 600 006. E-mail: tnlwb@vsnl.net. You can contact the above address or even try Seetharam & Bros, who are the authorized vendors for legal forms and formats.

Filled-in Form-A, along with the cheque, should be sent to the above address on or before 31st January of every year.
