Need a Simple "Referring a Friend" Form for Interviewees – Can You Help?

Can anyone send me a format or a form for "referring a friend"? This form will be distributed to the employees/candidates who come for the interview.

I know that there are different ways to get a referral from an employee, but I hope there are some forms that these employees or candidates who come for the interview can fill out if they want to "Refer A Friend".

We are running a "Refer A Friend" program in our company, wherein employees refer their friends for job openings. I wanted to inquire if there are any other methods or interesting templates/forms that I can implement to achieve better results.

I do not have a specific format, but I can tell you what information to include. You can add the requirements for the designations/posts along with some incentives depending on the designations.

For example:
Post of Assistant Manager,
Qualification - MBA or equivalent,
Incentive - if selected, $20,000.
