Seeking Help to Design Appraisal Forms for Hospital Technicians in Various Departments

I am working as an HR Professional at Ruby Hospital, based in the city of Pune, India. Can any HR Professional help me to design the appraisal form for Technicians in Pathology, Blood Bank, Transfusion Medicine, Cath Lab, Radiology dept, etc.?
