Hi Lavanya,

I did post a similar request here but at the end of it all, I realized that our requirements are usually very specific, hence we will have to create one for our needs individually. While other templates do serve as excellent mind-juice churners and give us ideas, they fail to pinpoint our need. Therefore, I wish you all the best. I am also attaching my survey copy to you for juice-churning. :-)

What is the questionnaire for? You may have to read reports and adopt the questionnaire from that to suit your needs. Like most bloggers, you have also requested questionnaires without giving enough information about the purpose, where you will use it, etc.
