Pl. enlighten me on the required forms to be submitted to get the death claim of a deceased member.

Greetings for the day!!!

Nominee has to file the claim in FORM 20 and needs the following documents :

EPF Claim FORM 20
Affidavit of Dependents in favour of Nominee
ID Proof of Dependents & Nominee
Declaration of nominee
Bank Details of Nominee

Kindly arrange all these before filing EPF death claim.
Pls find the attached FORM 20
