Confused About Filling ESI Form 37? Need Help with the Dates!

Dear all,

In ESI Form 37, what should be written in those blanks? Can anyone clarify which dates should be filled in those blanks?

(i) Has continued to be in employment/re-entered insurable employment on [date] and contributions have been payable/paid in respect of him/her during the contribution period which began on [date].

(ii) Has paid contributions for 78 days in the preceding contribution period which ended on [date].

First blank: you should mention the date of joining.
Second blank: you should mention the current contribution period starting date (1st Oct).
Third blank: should be filled with the previous contribution period end date (30th Sep) if he has contributed for more than 78 days. If not, then it is not applicable. Please put a cross mark for that point.
