Designing an Employee Satisfaction Survey: Can You Share a Helpful Template?

I work with an IT company that has BPO and KPO divisions as well. I am planning to design an "Employee Satisfaction Survey" for all the divisions of my company where I want to cover as many areas as possible, including departmental, administrative, operational, financial, and others, so that we can get an accurate picture and work on improving employee satisfaction.
