Exit interview Format - initiate suitable remedial steps to arrest the employees severance

We are in the service industry. I plan to introduce the 'exit interview' and initiate suitable remedial steps to arrest the employees severance from the org.
I request good exit interview forms to enable me to design one for our Org.

I wanted to ask if someone has a template of Exit Interview Tracker Excel Spreadsheet. I have the Exit Interview drafted and have been sending them. But I wanted to maintain a spreadsheet of Employees once I receive them back. If anyone can help, I would really appreciate it.
