How Can We Use Exit Interviews to Understand and Reduce Employee Turnover?

We are in the service industry. I plan to introduce the 'exit interview' and initiate suitable remedial steps to prevent employees from leaving the organization. I request good exit interview forms to enable me to design one for our organization. Please send formats.
