hi am trying to design an interview evluation sheet for my company,kindly advise on format and what to evaluate on.

U can decide the various common parametres which r generally required for all posts like, communication skills, presentation skills, technical skills etc. & then rate the candidate as per his skill levels!

Dear Nalini,
The format you have sent is fine and contains most of the attributes for assessing suitability of a candidate during an interview.
However, it would be a better idea to have one sheet for each candidate. One sheet is always easier to preserve in the personal file.
