What is the purpose of the survey? How many responses are you expecting? Are you surveying your own staff or are you expecting members to respond? Do you want to conduct any statistical tests or are you going to just use descriptive statistics?

There are more questions that need to be answered before people can give you suggestions. Also, please proofread the questionnaire to eliminate errors which a "Spell & grammar check" will not identify.

This survey/questionnaire was used for the production floor employees of NTPC in the year 2006. The analysis has been submitted to the NTPC HRD for further processing and improvement. I am sharing the questionnaire here only.

Sir, I am doing research work in Nagpur, India, at a manufacturing company. My hypothesis is whether the WPM Scheme influences employee motivation. I have collected data for this purpose. However, to fulfill the objectives and test the hypothesis, I need to design both closed and open-ended questions. Kindly guide me in this process.

I am a bit puzzled. Why are you conducting this research? If you have already collected data, could you please share what data you have gathered? Have you conducted a literature search to understand what others have done, how, and why they did it that way? What are the project's objectives and your hypotheses? We require this information before we can advise you on designing a questionnaire.

Many students fail to do their homework and then encounter difficulties.

My hypothesis is "Workers' participation scheme influences the motivation of employees of a manufacturing company (i.e., ABC)." For that, I have collected secondary data, and for the primary data, I have to design a questionnaire. So kindly guide me on the same.

Regarding your statement "This survey/questionnaire was used for the Production floor employees of the NTPC in the year 2006," I have a query. Do you work at NTPC? NTPC (or any Power/Energy sector company, for that matter) doesn't have a department called 'Production.' There are only Erection, O&M (Operation & Maintenance), and a few other departments at the Plant-end.
