Navigating ESI Issues: How to Handle a Lost Card and Missing Insurance Number?

I have two problems related to ESI.

Problem 1:
One of our employees lost the ESI Permanent Card.

Problem 2:
One of our employees joined three years ago and is still with us. We have been deducting the amount and remitting it, but he does not have an Insurance Number. He is now asking me for an ESI card. I am new to this organization. Is it possible to apply the declaration form with the same joining date?

1. You can apply for a duplicate card through Form 72.
2. You may not get the same date of entry, but ask for suggestions from the ESIC branch manager.

Dear Siva,

Issue a letter of employment to your employee with the insurance number mentioned in it. The employee has to produce the letter along with his family photograph to the ESI branch and obtain the duplicate card.

A letter describing the delay in submission of the declaration form needs to be submitted along with the DF. The employee would be allotted the number, and the card will be given to him.

I have been working with Apex in my old organization, and I handed over my ESI card there. Now that I have joined a new organization, I need to apply for a new ESI card. How do I go about this process?
