Lost Esi Permanent Card

Dear Friends,
I have two problem related to ESI
Problem 1
One of our employee lost the ESI Permanent Card

Problem 2
One of our employee was joined before three years still he continue. We are deducted amount also remitted but he doesn't have Insurance Number. Right now he asking me for esi card, i newly enter this org. Is it possible to apply declartion form as same date joining date......

1. You can apply for duplicate card thru form 72
2. u may not get same date of entry, but ask the suggetion from ESIC branch manager.

(1) Issue a letter of employment to your employee, with the insurance number mentioned in it. The employee has to produce the letter along with his family photograph to the ESI branch, and obtain the duplicate card.
(2) A letter describing the delay in submission of the declaration form needs to be submitted along with the DF. The employee would be allotted the number and the card will be given to him

I Have working apex but old organization esi card hand over,.but new organization.new join how esi pentameter card apply.
