How Can I Combine My Previous Pensionable Service with My Current Job Benefits?

I want to clarify some of my doubts regarding Employees Pension Scheme 1995.

Sir, I have worked in a Govt. Undertaking for 18 years, i.e., from April-82 to Oct-2000, and in the year Oct-2000, I took voluntary retirement. At that time, my pensionable service was 5 years. I have also received a Scheme Certificate from the Asst. Commissioner of PF (Pension), Raipur, stating 5 years of pensionable service.

Now, I have joined another organization where also PF 1952 & EPF Scheme 1995 are applicable, against which I will be eligible to receive a pension after my superannuation. I want to know how I can add this pensionable service of 5 years to my present service, which is ongoing.

Regarding the transfer of previous 18 years (don't restrict on pensionable service of 5 years, the past service is also transferable), please contact the Office of the Regional PF Commissioner. If you furnish the following details regarding the previous job as under, I shall inform you to what extent you have to contribute to the pension fund and what your return will be:

1. Date of Birth
2. Date of Joining
3. Date of Exit
4. Salary on 16.11.95 and at the time of exit from EPS
5. Break in service if any, before and after 16.11.95
