Various types of leave may include Annual leave, Sick leave, Compensatory leave, Educational leave, Casual leave, etc. The form may contain all of the above options or whichever is suitable for your organization as the title. The form should include the following details:

1) Employee's Name
2) Employee No. (if applicable)
3) Department Name
4) Employee's Designation
5) Leave from: ____ to ____
6) Number of Days
7) Resume duty on: ______
8) Leave Address or Phone No.: ________
9) Employee's Signature & Date
10) Approving Authority's Signature and Date
11) For office use: Serial No. (if applicable), Data posted by, etc.

By structuring a form with the provided details and potentially making improvements, you can create a more effective Leave Application Form.

Respected Sir,

I want to know about the maternity leave during probation (after recruitment as a Permanent Lecturer in college). I took maternity leave in my first year of probation.

1. Will it affect my seniority?
2. Do I have the right to full wages during my maternity leave?
3. Is it possible for my employer to consider my leave as Leave without pay instead of Maternity leave?

I have taken this leave for the first time solely based on maternity, but my employer college considers it as Leave without pay. If there are any guidelines from the Supreme Court or decisions regarding this matter, please guide me. Oblige. C. T. Pathak
