I have joined a new establishment, and we need to register the company for PF, ESIC, Labour Welfare Fund, etc. I am requesting you to please let me know the list of documents required for the registration of PF, ESIC, and Labour Welfare Fund.

Hi Anuradha,

Warm greetings.

As per my knowledge, you need to prepare the below-mentioned documents:

For ESI:
1. Memorandum of Articles
2. List of Board of Directors
3. Factory/company license
4. Sale deed/khata
5. Bank account details
6. Details of employees
7. Nomination forms in Form 2

For EPF:
1. Form 5A
2. Form 13 - Proforma of coverage
3. PAN Copy
4. KST & CST
5. Deed
6. Particulars of employees

It is not mandatory, but sometimes the PF department will ask just for verification purposes; they might ask. It's better to keep it ready. They will cross-check the wage details by comparing the balance sheet and wage details provided by you.

You need the following documents for registration for PF & ESI:

For PF:

- Business Number Allotment Form (may be obtained from PF Office)
- List of minimum 20 Employees with their Date of Joining and Rate of Salary
- Memorandum & Articles of Association with Certificate of Incorporation/Partnership Deed as applicable
- Copy of PAN
- Bank account existing Certificate
- Copy of VAT Registration
- Copy of Factory/Shop & Commercial Establishment Registration
- DD/Cheque for One month advance contribution (Optional)
- Copy of any other Registration as may be required

For ESI:

- Form 01 Employer's Registration (may be obtained from ESI Office)
- List of minimum 10/20 Employees with their Date of Joining and Rate of Salary
- Memorandum & Articles of Association with Certificate of Incorporation/Partnership Deed as applicable
- Copy of PAN
- Bank account existing Certificate
- Copy of VAT Registration
- Copy of Factory/Shop & Commercial Establishment Registration
- DD/Cheque for Six month advance contribution (Optional)
- Copy of any other Registration as may be required
- Declaration Form of each employee along with their 2 Postcard size photographs (Optional)
