Requesting you to please let know the list of documents required of registeration of PF, ESIC and Labout welfare fund.

For ESI
1. Memorandom of articles
2. List of board of directors
3. Factory/company licence
4. Sale deed/ khata
5. Bank acc details
6.Details of employees
7.Nomination forms in form 2
For EPF
1.Form 5A
2.Form 13 Proforma of coverage
3.PAN Copy
4.KST & CST
5.Deed
6.Perticulers of employees

It is not mandatory but some times PF department will ask just for verification purpose they might ask. Better keep it ready. They will just cross check with the wage details by comparing Balance sheet & Wage details given by you.

You need following documents for registration for Pf & ESI

For PF

Business Number Allotment Form (may be abtained from Pf Office)
List of minimum 20 Employees with their DOJ and Rate of Salary
Memrandum & Articles of Association with Certificate of Incorporation/Partnersheep Deed as applicable.
Copy of PAN
Bank a/c existing Certificate
Copy of Vat Registration
Copy of Factory/Shop & Comm. Est Registration
DD/Cheque for One month advance contribution (Optional)
Copy of any other Registration as may be.

For ESI

Form 01 Employer's Registration (may be abtained from ESI Office)
List of minimum 10/20 Employees with their DOJ and Rate of Salary
Memrandum & Articles of Association with Certificate of Incorporation/Partnersheep Deed as applicable.
Copy of PAN
Bank a/c existing Certificate
Copy of Vat Registration
Copy of Factory/Shop & Comm. Est Registration
DD/Cheque for Six month advance contribution (Optional)
Copy of any other Registration as may be.
Declaration Form of each employees along with their 2 Postcard size photographs (Optional)
