I have extensively searched this site about the documents to be submitted by the Contractor to the Principal Employer, and I would like some clarifications regarding the following:
For Appointing Contractor
1. Copy of contractor license
2. PF Registration number
3. ESIC number
For PF
1. Copy of Form 5 to show new employees (monthly)
2. Copy of Form 10 to show leaving employees (monthly)
3. Copy of challans with Bank Stamp (monthly)
4. Copy of Form 3A: details of wages & contributions for each member (Annual)
5. Copy of Form 6A: consolidated details of wages & contributions statement (Annual)
For ESIC
1. Photocopy of temp ESIC Card within 3 Days
2. Photocopy of permanent ESIC Card within 3 months
3. ESIC Half-yearly returns copy
4. Copy of ESIC amount remittance of the previous month with the current bill
Registers to be Checked
1. Attendance registers
2. Wages paid registers (for cash & bank)
I would appreciate it if you could provide clarity on the same.
Are the above requirements in order? Seniors and experts, please do guide me.
