Confused About Filling Form 10C? How to Handle Missing Employer Info for PF Submission

I do not know about these fields. Can I leave these fields blank? If not, from where can I get this information? I am not in touch with my last employer and have relocated. My last employer does not reply to my PF queries. Can I submit form 10C after leaving these fields blank to the PF office directly?

These details are available in your PF No. For example, if your PF No. is MH/19900/xxxx, MH is the region code, i.e., Maharashtra, 19900 is the Establishment code, i.e., the code number of your employer. "XXXX" represents your EPF No. The A/C No. is your bank account number.

You cannot leave these fields blank because your claim cannot be processed without them.

Region - stands for the State such as MH - Maharashtra, UP - Uttar Pradesh.

SRO - Sub-regional office, for example, in Maharashtra, Bandra (BAN) and in Uttar Pradesh, Ghaziabad (GHZ).

EST CODE - Code allotted to the establishment (Usually a five-digit code - 18960).

A/C No. - Usually a four-digit code - 2136, which is your PF account code allotted to you by the employer.
