Struggling with ESIC Sub-Code Registration? Need Help Understanding Form C11?

Please advise me regarding ESIC sub-code registration. I am stuck because I do not know about Form C11. Can anyone provide me with a format of Form C11?

Form C11 will be issued at the time of your ESI registration. When applying for a subcode, they will ask for a copy of C11. Please inquire at the ESI head office to get a better understanding of the forms required for obtaining a subcode. :)

Thank you very much. However, I have one more doubt: Can the contributions already paid at the Head Office for all employees located in various branches be transferred to their respective branches' ESIC after obtaining the subcode, or do I have to pay them again.

You need to prepare the below-mentioned documents:

1. Code allotment copy (Form C11)
2. Memorandum of Articles
3. List of board of directors
4. Factory/company license
5. Sale deed/Khata
6. Bank account details

Then you can apply for a sub-code.
