How Should I Update This Salary Certificate Format for a Bank Loan Request?

I have a format of a salary certificate. One of our employees is asking for a "Salary Certificate" to submit to the bank for a vehicle loan purpose. I have provided the format here. However, my senior suggests that I add a specific sentence indicating that the company is not responsible for any financial matters between the employee and the bank (e.g., pending installments deducted from salary, or any other matter) and that the certificate is issued only upon the employee's request.

Please provide the format as it is urgent.

A small tip: I am not good at English and it is better than having poor English. The following steps may be useful:

1. Know about English grammar.
2. Understand when to use words in different contexts.
3. Do not translate your English from your mother tongue or any other language.
4. You should understand other languages and translate them into English.
5. Read English books and watch movies.
6. Write comments in your notes, etc.

Myself: My English is very poor. After my postgraduate course, I took English coaching, but my English did not improve. After joining an organization, I continuously wrote reports in English. Then, someone would correct my letters and point out my mistakes. This is just my personal experience.

Probably what you can do is start reading small articles in English newspapers. You have to read aloud, at least to the extent that you can improve your pronunciations. Also, start referring to the Oxford Pocket Dictionary for queries. It really helps. Try speaking in English as much as possible at your home, with friends, etc. Even if you make a mistake, it's fine. Ask them to correct you so that you can learn faster. At this stage, don't worry about what people would say.
