I hope the attached file will help. If you know Excel, you could customize it to the current year and your company's requirements of permitted leaves,etc. Or you could take help from any friendly Accounts guy, they are usually experts in Excel.

This is good but i need a format to enter the timein & time out details daily & Movement register details to put in soft copy.
