Need an Excel Format for Cash Conveyance Allowance: Can You Help Quickly?

Hi,

Could anybody send me the separate format of giving conveyance allowance in cash in Excel so that I can manage the records of conveyance allowance and get it signed by my employees? The format must include the number of days present on the field, charges for 2 wheelers or 4 wheelers which automatically conclude their conveyance charges, and other important criteria.
