It's simply superb, but if you include a sample slide of a PA for a particular position with explanations, it will be easier to understand the same.

Please find a performance appraisal form designed by us in our organization. See if you can benefit from that. Suggest if I can improve to cover a few more inputs (ignore spelling mistakes which I have already noticed in that). I froze it as I felt a 360-degree appraisal will be too much for them now.
