I am attaching the DISC Profile Questionnaire and Key. I have a few questions for you:

1. Could you please confirm if it is correct?
2. What is the step-by-step process for calculating the DISC score and creating the graph?
3. If anyone is familiar with reading the graph, kindly let me know.

(Symbols are used with Wingdings and Wingdings 3 fonts; the same is inserted as comments at the bottom. If not supported, please change the font accordingly.)
