How Can I Improve My IT and HR Effectiveness Questionnaires? Your Feedback Needed

I'm currently working on my project. The aim of the project is to see how IT is contributing to HR effectiveness. I have done 2 questionnaires: one for end users and one for managers.

Please feel free to give your comments on the questionnaires. I would be grateful if you could suggest some questions which I can add.
