I am uploading the format for Exit interview used in our company...
I think it would be beneficial to the community...

I would be interested to know what happens to the form after the employee completes it. Does it go straight to the manager?
I imagine employees will be prepared to give more critical feedback if appropriate if they know that the manager won't have the opportunity to read the form before they go.
The organisation I previously worked at had the Manager conduct the exit interview, which seemed like a realy bad idea to me. Nobody gave any kind of meaningful constructive criticism as a result, but later when meeting people socially, they would give plenty of detail about how bad the system/manager was .

Thats the bad part...
The second part of the form is currently being filled by the reporting manager. Which is absolutely wrong practice...
I totally agree with you that the filled in form should be kept secret and passed on to the senior managers / research personnel after the candidate has actually left the company with all dues cleared..
But as of now, the senior management is able to get the correct information [not always reliable] through grapevine.. and employee counselling...

Sounds the same as my last organisation. Still, changes can be implemented over time as people experiment with the process. Good luck.

I forgot to say that I think the content of the exit interview form you put up is excellent. This is the kind of information I would value from exiting employees, if honestly answered.
