Struggling to Find the Professional Tax Challan Format? Can Anyone Help?

I have seen many friends asking repeatedly for the Professional Tax Challan, and I too am in urgent need of it. However, I have only seen PF and ESI Challans posted. I tried to search and download it too, but could not find it :(. If anyone has the format, kindly upload it.

Kindly forward me the latest PT Challan, because when I sent Form 5A to SBI bank, they are asking for a new challan form with 4 numbers.
