This is really nice and will help quite many HR professionals emphasizing on PMS. However the only problem I see in the form is the inclusion of too many competencies/behavioural traits.. In any robust PM system is is advisable that u dont have more than 6-8 KRAs and not more than 8 competencies. because it then becomes difficult for any individual to display those many positive behaviours..
