This is really nice and will help many HR professionals in emphasizing on PMS. However, the only problem I see in the form is the inclusion of too many competencies/behavioral traits. In any robust PM system, it is advisable that you don't have more than 6-8 KRAs and not more than 8 competencies because it then becomes difficult for any individual to display that many positive behaviors.
