Dear Anju,

This is really nice and will help many HR professionals in emphasizing on PMS. However, the only problem I see in the form is the inclusion of too many competencies/behavioral traits. In any robust PM system, it is advisable that you don't have more than 6-8 KRAs and not more than 8 competencies because it then becomes difficult for any individual to display that many positive behaviors.

Your model performance appraisal template is very nice and useful. However, it is too elaborate. Could you please condense it so that management can easily review and fill it out without losing patience? Overall, it is good.
