Need Help with Shops & Establishment Act Forms for Orissa and Uttarakhand – Can You Share PF Templates Too?

Can somebody help me with the statutory forms and templates related to the Shops & Establishment Act for Orissa and Uttarakhand? It would also be helpful if anyone could share the PF forms specific to the state (Form 2).

Templates pertaining to labor/S & E like in TN, there are Form Q, Form P, etc. I would like to have similar templates for Orissa & Uttarakhand. Also, I would like to get a scanned copy of Form 2 (PF employee nomination form). Is there someone who can help me with this? Thanks in advance!
