I have a suggestion which you might want to look at. Rather than having just one progress report prior to confirmation, if your probation period is 6 months, it would be more constructive to have a mid-term review at 3 months of a person joining. By then, if there are any major concerns regarding performance, it can be conveyed to the candidate, and an action plan drawn up to improve performance. This will avoid future hassles like an extension of probation and other legalities involved. Roma's form has captured all the essence of a progress report. You can use the same format for the mid-term review too.
