I'm working in a publishing company as an HR Executive. We have just finished our yearly appraisals last month. The employees of my company have been given specific objectives to achieve over time (many have monthly objectives). I have been tasked with designing a post-project appraisal form to track whether the objectives are being met or not, making it easier to take necessary actions if they are not achieved.
Can someone please help me with designing the form? Please respond as soon as possible as this is one of my objectives, and I have never designed such a form before.
