Hi Sujatha,
We have 3 models of appraisals:
1. 180
2. 360
3. 540
Please find the appraisal formats attached. Since you do not require 540, I have not attached it.
540 entails the client also giving their comments on the resource.
The immediate boss, who is the reporting lead, would provide their comments in 180.
In 360, both the immediate boss and their boss would give comments.
