Starting a New Plant: How Do I Register for PF Coverage Under the Factory Act 1948?

Dear All,

I am in a manufacturing unit, and we are starting a new plant. Can anyone please send me the proforma for coverage under the PF under the Factory Act 1948? I need more information regarding this. Could you please suggest to me how to complete the registration formalities?
